        <li>Understand that it is a legal requirement for the testing laboratory to notify the federal, state  and/or local public health authorities in the event that certain diseases (such as Chlamydia) are detected in a urine sample</li>
        <li>Understand that, where indicated, we may recommend that you contact a health your health care provider in connection with your results and it is your responsibility to follow this advice if provided        </li>
        <li>Confirm that the sample collection appointment process has been explained to you in full and understand that you will receive your results via Thryv  - [and that your results will be provided to the health care provider who ordered your test] ;</li>

            You can withdraw consent at any time, without reason. Please be aware that this may restrict or stop what Randox Health can provide as part of your programme.  </li>

    I consent that Randox Laboratories may store and use any surplus biological samples provided for future research and development and/or Quality Control purposes. All samples will be pseudonymised and will be disposed of once the research purpose is fulfilled. You are entitled to withdraw your consent at any time, following which, samples shall be destroyed.
